Dr. Schulz welcomes referrals from physicians throughout the state of Iowa. If you have a patient who is in need of an evaluation for venous disease, varicose veins or spider veins please print the following Physician Referral Form and fax to Infinity Vein Center at: 319-337-5361. If this is not possible, please complete the Referral Form and give a copy to the patient so he or she can bring the completed form with them to their appointment.
